Get the kids outside and active in the next school holidays

Wednesday 3rd October 2012, No 1 Oval Tamworth
Thursday 4th October 2012, Sports UNE, Armidale
7 to 12 years
8.30 am (signing in) 9am to 3.00 pm
Cost:
Includes morning & afternoon tea and lunch:
Early bird price $40 - closes Friday 14 September
Regular price $50 or $90 for two children

Aboriginal & Torres Strait Islander participation grants available for financially disadvantaged familiers. See your school or contact NIAS for further information.

Program Includes: Cricket, Football, Netball, Rugby League, Rugby Union, Softball, Running, Ultimate Frisbee, Obstacle Course and more fun activities.

** Shade and water will be made available however please ensure you provide sunscreen and a hat for your child.
*** All kids receive a Frisbee and Drink Bottle

Medical and Waiver Details:
By completing the below registration and payment, in the event of accident or illness I authorise the obtaining on my behalf such medical assistance as may be required for me (or my son/daughter in case of under 18). I also authorise the administering of anaesthetic if this is deemed necessary by the Medical Officer attending.

I understand that NIAS will not meet the costs of any medical, dental or hospital service required, and that it is my responsibility to ensure that I have adequate private health insurance and extra cover at all times. I also indemnify any presenters or instructors involved by NIAS and acknowldege that I (or my child, if under 18) is in a fit and healthy state to take part in the camp.

I give permission for NIAS to take photographs of my child/children for the purpose of marketing. Please contact the NIAS Office if you do not wish your child/children to be photographed. 6766 2526.

Registration and Payment:
You can register and pay easily by selecting the camp you wish to attend, add childs name and date of birth, emergency mobile phone contact no during camp and click on the below "add to Cart". If you wish to add a child or register for both days simply then click on continue shopping. This will take you back to this screen, change camp option below and click add to cart again. Once finished you can proceed to checkout to complete transaction. When you proceed to checkout you will be entering a secure area carrying a security certificate to encrypt all your personal details, including your credit card information, your name, phone and address. You will receive an email confirmation of registration and payment. We then look forward to seeing you and your child/children at one of our "NIAS Junior Activities Days.
